Key Responsibilities
  • Provide on-site patient care using a harm reduction approach
  • Focus on HIV, Hepatitis C, and chronic condition management
  • Conduct assessments, diagnoses, and treatment planning
  • Prescribe and manage medications
  • Educate patients on prevention and wellness
  • Collaborate with physicians and interdisciplinary teams
  • Maintain accurate documentation in EMRs
  • Follow CNO standards and privacy regulations
  • Engage local health and community organizations
Qualifications
