Assisted Living Alberta is dedicated to Enhancing Care in the Community, helping people be healthy, well & independent in their homes & communities. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Registered Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages & stages of life. As a self‑directed team member within Continuing Care in the Edmonton zone, the Registered Nurse works collaboratively with the health care team & has responsibilities, including assessment of needs, delivering appropriate therapeutic nursing services within professional practice standards, coordinating resources, establishing goals for client delivery, & evaluating outcomes.

In addition, this position provides case management as well as direct client care that meets established clinical standards, is evidence based & client centered. In collaboration with family & other partners, the RN acts as a consultant on the team providing clinical expertise & direction to other team members regarding client cases, developing a care plan, based on those unmet needs, using the appropriate internal/external resources. As a member of the multidisciplinary team, the RN is responsible for participating in staff development & mentoring student clinical experiences. This position facilitates complex discharges from Acute Care to the Home & works collaboratively with Acute Care Transition Coordinators & multidisciplinary teams within Continuing Care & develops recommendations to support client flow to Home Living options. The Mental Health System Case Manager provides consultation & system navigation for Home Living clients living with mental health or addiction symptoms.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well‑being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications
  • Completion of an accredited nursing education program.
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS‑HCP) certification.

Additional Required Qualifications
  • Two years of recent nursing experience (obtained within the last 5 years) or completion of a refresher program within 2 years.
  • Self‑directed and the ability to work independently and use sound professional judgement.
  • Excellent organizational skills – to be able to prioritize and efficiently organize many details in a timely manner.
  • Communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast‑paced environment.
  • At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands‑on assessment.
  • You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.
  • Physically capable of performing nursing tasks which could include transfer of clients.
  • The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk‑up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25‑30 lbs.
Preferred Qualifications
  • Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered.
  • Palliative care experience.
  • Previous Continuing Care and/or geriatric experience.
  • Experience with Connect Care and the RAI Home Care assessment tool.
  • 2 years current Case Management Experience in Continuing Care.
