Job description

Position: Registered Nurse

Status: Temporary Full-Time (Maternity Leave Coverage)

Schedule: 12-hour Day shifts (6am-6pm)

Location (Home): Heartwood LTC, Cornwall, ON

Southbridge Care Homes is seeking a caring, passionate, and enthusiastic Registered Nurse to join our Heartwood family. In this role, you will play a vital part in supporting our lovely residents, ensuring their safety, health, and happiness every day. You will help maintain the highest standards of nursing practice, aligned with regulatory requirements and Southbridge’s commitment to exceptional care.

Reporting to the Director of Care, the RN will enhance the effective management and quality of nursing practice in keeping with the Standards of the College of Nurses of Ontario, Long Term Care Standards and Southbridge Standards.

Key Responsibilities
  • Deliver resident care in accordance with established nursing standards, physicians’ orders, facility policies, and best practices.
  • Monitor, observe, report, and document residents’ vital signs, symptoms, and overall condition.
  • Assess residents and develop, implement, review, and evaluate individualized care plans.
  • Monitor residents’ health status, recognize changes, and facilitate timely physician referrals; process and execute physician orders.
  • Plan, supervise, and evaluate nursing and personal care staff performance on the unit.
  • Orient, train, and support new staff; contribute to ongoing staff education and professional development.
  • Conduct performance appraisals and address staff concerns, referring issues to the Director of Care as appropriate.
  • Assist with preparing the facility for RAI MDS assessments.
  • Assume charge nurse responsibilities in the absence of the Director of Care.
  • Provide staff coverage for care clerk duties as needed.
Qualifications
  • Current Certificate of Competence from the College of Nurses of Ontario (CNO) required.
  • Registered or eligible for registration with the Registered Nurses Association of Ontario (RNAO).
  • Previous managerial experience in long‑term care preferred.
  • Experience working with cognitively impaired residents.
  • Strong knowledge of nursing practice, health care standards, and infection control protocols.
  • Demonstrated commitment and compassion for working with seniors.
  • Ability to prioritize urgent tasks and work effectively both independently and within a team.
  • Excellent communication skills, both verbal and written, with an approachable and professional demeanor.
  • Strong critical thinking skills, able to analyze information from multiple sources to develop effective solutions.
  • Excellent organizational, planning, time management, and multitasking abilities.
