Job Description
The Ontario Shores seeks a Registered Practical Nurse for the Assertive Community Treatment Team (HKPR). The successful candidate will have sound knowledge of clinical issues in the area of mental health and addiction and will report to the Clinical Manager.
Responsibilities
- Mental health and physical assessments
- Participate as a mental health professional on a multidisciplinary team
- Provide assertive community treatment to clients with severe and persistent mental illness
- Provide client‑centred treatment rehabilitation and support to referred clients
- Develop treatment and rehabilitation goals and plans in collaboration with clients, families, support network and multidisciplinary team
- Provide clinical case‑management services, including psychosocial interventions, environmental adaptations, medication delivery, monitoring medication compliance, assistance with activities of daily living, harm reduction related to substance use, vocational rehabilitation, social relationship and leisure time use
- Assess for and recognize signs of deterioration and institute actions to prevent further hospitalisation
- Provide emergency crisis intervention services to help the client and/or others (e.g. family members, home operators, emergency room physicians) manage emotional or behavioural crises with other clinicians as appropriate
Requirements
- Registration with the College of Nurses of Ontario in good standing
- Current CPR – Health Care Provider (HCP)
- Demonstrated knowledge of CNO Standards of Practice and commitment to maintaining reflective practice standards
- Demonstrated knowledge of Canadian Standards for Psychiatric – Mental Health Nursing and relevant policies and legislation, such as the Mental Health Act and PHIPA
- Proven excellent interpersonal skills and effective oral/written communication skills
- Implies and models the Recovery philosophy in all aspects of care as a Recovery Champion
- Demonstrated working knowledge/experience with computers
- Meditech Experience preferred
- In‑depth knowledge and experience of the challenges of acute psychiatry
- Skill and knowledge in the area of psychosocial rehabilitation and recovery
- Preferred: mental health experience of one year or more
- Ability to work independently and within a multidisciplinary team
- Understanding of recovery principles and their application to people with mental illness both in hospital and community settings
- Capacity to work with clients of diverse ethno‑racial and cultural backgrounds; proficiency in a second language considered an asset
- Proven good attendance record with the capability of maintaining this standard
- Must be able to work all shifts and weekends
