Overview
In this community nursing role, as a Part-Time Night-Shift Registered Nurse (RN) you will have scheduled visits between the hours of 10pm - 8am to care for patients in the Waterloo Region. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

What You’ll Do
- Using appropriate assessment tools/techniques to assess patients’ individualized needs
- Executing and continuously monitoring care plans
- Engaging patients using a collaborative approach by building rapport and trust
- Maintaining and updating patient health records related to encounters, interventions and plans
- Ensure care is provided with established nursing standards and principles, physicians’ orders and administrative policies, as well as observe, report and record vital signs, symptoms and conditions of residents / patients
- Ability to collect and use information to thoroughly think through problems and situations; puts information from various sources together to see the ‘big picture’ and generate creative solutions
- Ability to work alone and with others while prioritizing tasks to handle the most urgent first
What You Bring
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver’s license and access to a registered vehicle is required
- Palliative care experience is an asset
- Post-graduate education in Palliative care is mandatory
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Passion for making a difference in your community
- Ability to present information clearly and effectively both verbally whether in person or over the phone, and in writing; an active listener who projects a friendly demeanour
