Job description

Job Description: Licensed Practical Nurse (LPN) - Home Care/Hospice

Provide professional nursing care under the direction of a RN Case Manager and/or the Clinical Manager.

  1. Perform skilled nursing visits for patients as directed by the plan of care developed by the RN Case Manager, following established care plan interventions.
  2. Recognize and report changes in patient condition to the RN Case Manager and/or the Clinical Manager promptly.
  3. Communicate information timely, in accordance with agency policies and the urgency of the situation.
  4. Provide care within the scope of clinical practice and competency, including assessment of patient status (physical, psychological, environmental), medication monitoring and administration, patient/family teaching, and emergency interventions.
  5. Deliver skilled nursing services in homes, schools, clinics, and other community settings.
  6. Provide on-call support for patients based on agency needs, rotating weekends, evenings, and holidays to ensure adequate staffing.
  7. Demonstrate competency in nursing skills according to legal scope of practice and agency policies.
Qualifications:
  • Graduate of an approved, accredited school of professional nursing; current licensure as a Licensed Practical Nurse (LPN) in Massachusetts.
  • Current American Heart Association Basic Life Support (BLS) certification.
  • Preferably, one (1) year of experience in home care/hospice or equivalent; knowledge of homecare regulatory and reimbursement requirements.
  • Strong written and verbal communication skills, interpersonal skills, organizational and time management abilities.
  • Valid driver's license, reliable transportation, and a good driving record.
