About The RoleThe Licensed Practical Nurse (LPN) reports to the Director of Care or designate. This role models and leads a team within an assigned community, coordinating care delivery for residents. The LPN performs duties within their scope of practice, adhering to standards, regulations, policies, and procedures to ensure resident-centered care.
ResponsibilitiesInclude but are not limited to:
- Develop and maintain professional relationships that support organizational culture and purpose.
- Assess residents’ needs; implement and evaluate individualized care plans.
- Ensure residents' rights are observed and promote a holistic approach to wellness.
- Liaise with physicians, community agencies, families, visitors, volunteers, and other disciplines to ensure quality care.
- Assess and treat wounds; act as a resource for care staff on skin integrity strategies; consult with RN or designate as needed.
- Administer prescribed medications and treatments within the scope of practice defined by BCCNM.
- Monitor and report on changes in residents' physical and psychological conditions; document and notify relevant parties.
- Participate in Interdisciplinary Care Conferences and care plan reviews.
- Support administrative tasks such as admissions, discharges, and confidentiality maintenance.
- Participate in staff orientation, community programs, meetings, committees, and educational sessions.
- Provide coaching and guidance to care staff; complete Point of Care documentation; seek assistance from RN or designate when necessary.
- Stay informed about community policies, emergency procedures, and safe work practices.
- Perform other related duties as required.
Qualifications And Skills- Graduate of an approved Practical Nursing program and current license with BCCNM.
- Valid Occupational First Aid Level 1 certification.
- Successful completion of annual RAI competency testing.
- Experience in long-term care is an asset.
- Must pass a vulnerable sector Criminal Record Check as per provincial guidelines.
- Proficient in English, with good reading, writing, and verbal skills.
- Computer skills including Word, Excel, Outlook.
- Ability to lift up to 25 lbs and work with cleaning supplies and equipment.
