Overview

The Director of Care Excellence provides strategic and thought leadership to advance quality, safety, and innovation in home and community care across Ontario. This role ensures compliance with regulatory, legislative, and contractual care standards, fosters interprofessional and inter‑portfolio collaboration, and drives evidence‑based, person‑centered care. Reporting to the Executive Director, Integrated, Specialized & Core Business Programs, this role works closely with Patient Care Operations and Shared Services portfolios to develop, translate, support operationalization and monitoring of clinical and integrated best practices that support optimal enterprise performance and outcomes. The Director champions clinical governance, professional development across various disciplines, and continuous improvement aligned with organizational values, priorities, and provincial health mandates.

What The Role Involves

Strategic Leadership

  • Develop and execute the organization’s care excellence strategy aligned with corporate goals.
  • Lead system‑wide standardization, optimization, and evaluation of clinical/care practices and protocols.
  • Contribute to strategic planning and governance initiatives that promote value‑based care through a systems lens, drive adoption of innovative care models and digital health solutions that support an improved care delivery experience and outcomes, showcasing the value of home and community care and CarePartners as a leader

Regulatory Compliance & Professional Standards

  • Ensure all regulated health professionals’ practice within their full scope per college standards.
  • Monitor compliance with Ontario legislation (e.g., Excellent Care for All Act, CNO, COTO etc.) and Home and Community Care Support Services requirements.
  • Act as a liaison with external regulatory bodies to advance and advocate for care practice in the homecare space

Quality & Patient Safety

  • Collaborate with quality and patient care teams to identify clinical practice trends, safety risks, and improvement opportunities
  • Lead quality improvement initiatives, audits, and risk mitigation strategies.
  • Participates in quality of care reviews, root cause analyses, clinical audits, and quality investigations to identify and mitigate clinical risks
  • Develop and implement clinical policies, procedures, and best practices.
  • Define and monitor key performance indicators (KPIs) for clinical services.

Care Excellence & Workforce Development

  • Lead collaboration among nursing, therapy, and personal support teams to deliver integrated, client‑focused care.
  • Develop and implement integrated care practice standards, education programs, and orientation to support staff competency and ongoing professional growth.
  • Facilitate program planning, development, execution and evaluation by continuously analyzing care needs to assist in on‑going planning, evaluation and enhancement of programs.
  • Foster a culture of accountability, mentorship, interprofessional collaboration, and continuous quality improvement.
  • Oversee clinical performance across the organization, using data and key performance indicators to drive quality, efficiency, and care improvements.
  • Partner with other internal protfolios including Patient Care Operations, HR, Learning & Development, Quality, Data Analytics etc. to support workforce planning, training, and care practice optimization.
  • Engage patients and caregivers to ensure care is culturally safe, inclusive, and responsive to community needs
  • Promote evidence‑based practice, innovative care models, and effective knowledge translation to enhance care and system outcomes including but not limited to patient and caregiver experience
  • Represent the organization in external committees and partnerships, strengthening relationships with regulatory bodies, community organizations, healthcare partners.

Relationship Management

  • Build strong partnerships with internal teams and external stakeholders.
  • Engage clients and families to ensure culturally safe, inclusive care
What You Bring
  • Master’s degree in Nursing or other related degree
  • Registered Nurse in good standing with the College of Nurses Ontario (CNO)
  • Additional certifications considered an asset (CNA certification)
  • A minimum of five (5) years of progressive leadership responsibility, influencing and collaborating with senior and executive level management along with key organizational partners in home care or community sector.
  • Deep understanding of Ontario’s healthcare system, regulatory frameworks and home care models
  • Strategic thinking and leadership skills to drive change and influence key stakeholders
  • An ability to leverage data to drive business insights and support solutions in‑line with CarePartners’ strategic priorities
  • Proven ability to lead system‑wide standardization and process improvement initiatives
  • Experience with data analysis, and strategic thinking, with an aptitude for interpreting and reporting on operational data.
  • Pro‑active, innovative and results‑oriented with strong analytical problem‑solving skills
  • High effectiveness in communication and coordination of cross‑functional teams
  • Exceptional interpersonal, presentation and public speaking skills
  • High effectiveness collaborating within indirect reporting structures
  • Experience with clinical documentation systems and digital health tools (CHRIS, EMR, HCC systems)
  • Proficiency in Microsoft Office Suite
