Summary

The Community Health Nurse (CHN) works within an interdisciplinary team at the Nursing Station using the primary health care model and cross-cultural nursing concepts to provide culturally appropriate nursing services to community members. This is a clinical position which cares for clients across their life span individually, in families and in other groups or associations. The CHN provides services in accordance with the Standards of Practice and Guidelines from the Ontario College of Nurses. This position requires flexible work hours and on-call rotations and reports directly to the Nurse-In-Charge (NIC).

Responsibilities
  1. Conducts thorough assessments of the physical and mental health status of patients at the Nursing Station.
    1. Works with the NIC, the community physician and a team of community health workers to deliver Health Canada’s mandated programs in the Wunnumin Lake Nursing Station.
    2. Interviews patients to obtain their health history ensuring that physical, psychological, social, spiritual, and cultural factors are considered.
    3. Conducts physical assessments across the life span, taking into consideration normal parameters of growth, development and aging.
    4. Assesses mental health status of patients including risk factors and coping abilities.
    5. Analyses assessment information in consultation with the community physician to determine a working diagnosis. Considers health statistics and other relevant information in this process.
    6. Identifies patient objectives, priorities and needs.
  2. Establishes a health care plan based on a comprehensive assessment process.
    1. Identifies appropriate and effective measures to address clients’ defined needs and problems.
    2. Provides required treatment and care that falls within nursing scope of practice in consultation with community physician and relevant medical and health professionals.
    3. Identifies relevant community resources and services to the extent they are available and assists clients to access these.
  3. Implements a health care plan and facilitates the delivery of health-related services.
    1. Follows the Ontario College of Nurses Practice Standards and Guidelines to provide safe and ethical nursing care.
    2. Provides health care and treatment services in daily Nursing Station operations as well as during emergency situations and after-hours call shifts.
    3. Treats acutely and chronically ill clients in consultation with the community physician as required.
    4. Provides nursing services to clients requiring in-home care when needed, and educates and supports the family/caregivers of clients. Conducts home visits when time permits, and on occasion, provides palliative in-home care.
    5. Implements measures to prevent and control infectious diseases.
    6. Maintains a patient log book with professional, accurate and up to date information. Submits monthly patient statistics to NIC, Health Director and the Shibogama Nursing Director.
    7. Maintains patient files in a timely manner following regulatory and legislative requirements as per Ontario College of Nurses’ practice standards regarding documentation.
    8. Records and reports required reportable data using SOAP charting (Subjective, Objective, Assessment and Plan) and appropriate documents, e.g. HIS, CHIRRP (Canadian Hospitals Injury Reporting and Prevention Program).
    9. Provides health education, health promotion and prevention services as time permits, e.g. developing and delivering presentations, speaking on local radio on a variety of topics, distributing printed materials and pamphlets, etc.
    10. Adheres to the community Oath of Confidentiality and to the Ontario College of Nurses guidelines regarding client Confidentiality.
  4. Evaluates the effectiveness of interventions.
    1. Identifies key indicators to use as an evaluative tool to recognize changes in clients’ health status.
    2. Contributes to the revision of health care plans by providing information pertaining to the outcomes of interventions.
    3. Applies knowledge gained from each case to plan future client treatment and care.
  5. Networks and maintains positive relationships with outside healthcare professionals as well as those in the community and other stakeholders.
    1. Collaborates with the NIC, community physician, and Health Director to ensure Nursing Station programs are effective and operate smoothly.
    2. Collaborates with community healthcare staff regarding clients and their best interests.
    3. Consults with, coordinates care and makes appropriate referrals of clients to other healthcare professionals.
  6. Professional development.
    1. Participates in training and other professional development initiatives to ensure nursing skills remain current.
    2. Maintains up to date certification in the courses required for all community nursing staff by Shibogama Health Authority.
Knowledge, Skills and Abilities
  • A Registered Nurse (RN) in good standing with the College of Nurses of Ontario
  • Two years experience in a First Nations community delivering community-based programs or equivalent
  • Strong clinical skills and knowledge in Health Canada’s mandated health care program areas
  • Good professional judgment
  • Advanced Cardiac Life Support (ACLS) and or International Trauma Life Support (ITLS); Trauma Nursing Core Course (TNCC); Cardio Pulmonary Resuscitation (CPR – Health Care Provider); Emergency Nurse Paediatric Care (ENPC) or Paediatric Advanced Life Support (PALS); and Immunization Certification, as a minimum
  • Good knowledge of the programs, regulations, and procedures of the First Nations and Inuit Health Branch, Health Canada
  • Ability to use standard Nursing Station medical equipment and to conduct standard laboratory tests
  • Skills and experience related to health education, promotion and prevention
  • Training or experience in crisis intervention, counselling and supporting individuals is an asset
  • Knowledge of and sensitivity to the traditions, culture and needs of First Nations people
  • Excellent verbal and written communication skills in English; fluency in Oji-Cree is an asset
  • Ability to manage time and workload effectively, and successfully adapt to changes in workload and work environment
  • Willing to participate in ongoing training and other professional development, and views learning as part of the job
  • Proficient in using Microsoft Office and e-mail
  • Ability to work flexible hours
  • Willingness and ability to travel
