At Humber River Health, the Clinical Manager plays a key role in providing operational leadership and direction in the delivery, availability and continuous improvement of interdisciplinary clinical team, and services that provides consistently high-quality, safe care in our Home Program and LTC Satellites with out Humber River Wilson invented Dialysis.
We currently have an exciting opportunity for an experienced and committedClinical Manager, Home Programs and LTC Satellites, to join our Nephrology Program.This individual will be responsible for the development, coordination and monitoring of activities that promote clinically effective and efficient patient care. The Clinical Manager will be responsible for the operational, financial and administrative success of our Home Program and LTC Satellites. Additionally, the Clinical Manager will direct, supervise and coordinate all operations, including clinical and administrative staffing/supervision, budgeting, strategic planning, marketing, physician relations, equipment purchases and upgrades and facility development as appropriate. This individual will own the planning, organization and direction of patient care activities in the Home Program and LTC Satellites. They will demonstrate leadership competencies and values through development and coaching of direct reports. The Clinical Manager will also monitor employee retention, recruitment and employee satisfaction. They will consult and collaborate with other health care team members to provide safe appropriate patient care, while ensuring a culture of accountability.
As an integral team member of the Nephrology Program, there is an opportunity to lead the Program by utilizing a chronic disease prevention and management (CDPM) and self-management approach and philosophy, as the Program continues to achieve the deliverables under the Ontario Renal Network, and strives to achieve the targets of the Ontario Renal Plan (ORN) III:
