We are currently seeking a dedicated and passionate individual to lead our Care Navigation and Volunteer Coordination efforts. This role plays a key part in advancing Lumacare’s strategic priorities by connecting seniors to essential support services, coordinating volunteer engagement, and promoting our programs within the community.

Reporting to the Supervisor, Healthier Living Centre, the successful candidate will oversee outreach initiatives, provide care navigation for seniors, manage volunteer recruitment and recognition programs, and support the administrative functions of the Healthier Living Centre (HLC).

Key Highlights:

Care Navigation & Community Outreach
  • Assists with planning of support services, as well as connecting clients to external resources, as appropriate. Provide service and care navigation support across different buildings in North York
  • Maintain ongoing case management for clients and caregivers, offering support as needed.
  • Identifies and participates in outreach activities, including networking events, partner events, job fairs, volunteer fairs, and community outreach events
  • Promote Lumacare programs and services to the public and partner organizations
  • Support Adult Day Programs and assist with client assessments interRAI CHA
Volunteer Coordination
  • Lead recruitment, onboarding, and screening of volunteers and student placements.
  • Conduct interviews, reference checks, and police background checks.
  • Refers screened volunteers and students to lead staff in various program / department areas for all Agency sites.
  • Act as the main liaison between volunteers / students and Lumacare program leads.
  • Communicate with school administrators and internal staff to monitor volunteer performance and hours.
  • Plan and execute volunteer recognition initiatives and appreciation events, annual luncheons.
  • Develop and distribute the volunteer newsletter.
Healthier Living Centre (HLC) Program Support
  • Provide administrative support for virtual and in-person HLC program
  • Open and monitor Zoom exercise classes
  • Record client attendance and maintain physical and digital records using systems such as AlayaCare and Excel
  • Assist with class set-up and registration for new clients
  • Respond to client inquiries via phone, email, or in person about programs and services
  • Conduct client check-in calls and provide relevant resource information
  • Participate in committees, working groups, and collaborative initiatives as required
Position Requirements
  • Education in Social Service Work, Social Work, Psychology, Sociology, or a related field (preferred).
  • Experience working with the frail and elderly population, and knowledge of community resources
  • Strong administrative skills including proficiency in Microsoft Office (Word, Excel, Outlook) and familiarity with client databases AlayaCare).
  • Experience organizing events and facilitating community partnerships is an asset.
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to work both independently and collaboratively in a dynamic team environment.
  • Must have own vehicle for work-related travel. The position will require travel within the community in a variety of sites across North York.
