Responsibilities
  • Responsible for assisting Case Manager(s) to achieve an appropriate synchronized discharge plan for our patients/clients.
  • Communicates with contracted providers and coordinates any necessary tasks to accomplish discharge goals.
Qualifications
  • Requirements include knowledge of the major healthcare companies basic benefits in regards to OPT care, DME, Home Health, SNF.
  • Knowledge of the local IPA/HMOs and their contracted providers, HHA and SNF.
  • Ability to prepare and assemble patient charts.
  • Ability to communicate effectively with families including preparing and distributing information from patients/families.
  • Excellent communication skills.
  • Knowledge of medical terminology.
  • Ability to organize and maintain files, schedules, and records.
  • Ability to screen, prioritize, and route telephone calls.
  • Basic computer knowledge and general office duties including copying, typing, filing, faxing, correspondence, telephone systems, and scheduling.
