Job Overview

GluCare Integrated Diabetes Center is seeking a dedicated DHA Nurse to join our clinical team in delivering exceptional patient care. The ideal candidate is compassionate, adaptable, and committed to providing high-quality nursing services in a modern, fast-paced environment. Experience in chronic disease management, patient education, and digital tools is a plus.

Key Responsibilities
  • Patient Care Support: Provide direct patient care, including vital sign checks, blood glucose monitoring, and patient observations.
  • Educate patients on diabetes management, including lifestyle tips and monitoring instructions, ensuring compliance with care plans.
  • Clinical Procedures Assistance: Prepare examination rooms and medical equipment, ensuring cleanliness and adherence to infection control standards.
  • Assist in the preparation and handling of equipment for procedures, wound care, and specimen collection.
  • Patient Education and Communication: Communicate effectively with patients, providing empathy and clarity to address their questions and concerns. Reinforce health education provided by physicians, encouraging self-care practices and adherence to treatment plans.
  • Documentation and Reporting: Accurately record and update patient information, treatment plans, and observations in electronic medical records (EMR). Report any changes in patient condition to the supervising nurse or physician promptly.
  • Operational and Safety Compliance: Adhere to all DHA regulations, nursing guidelines, and GluCare protocols to ensure the safety and well-being of patients and staff. Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support.
Qualifications
  • Valid DHA Nurse License – A MUST.
  • Minimum 1-2 years of experience in a clinical setting, preferably in endocrinology, diabetes care, or chronic disease management.
  • Knowledge of diabetes management and patient education in lifestyle-related chronic conditions is preferred.
  • Strong communication and interpersonal skills with a patient-centric approach.
