Overview
The Social Worker will work closely with youths and their families within the community, providing holistic case management, counselling, and support services. This role involves outreach, assessment, intervention planning, and collaboration with stakeholders to address risks, needs, and opportunities for positive change.
KEY RESPONSIBILITIES
- Outreach & Engagement
Initiate and participate in outreach initiatives to promote Division programmes.
Develop outreach plans, establish partnerships, and maintain strong relationships with community stakeholders.
- Referral & Intake Management
Handle incoming referrals and liaise with referral sources for intake and assessment.
Conduct thorough assessments, gathering information from clients, families, and stakeholders to inform case conceptualisation.
- Case Management & Intervention
Collaborate with clients to co-create realistic and achievable intervention plans.
Provide counselling, emotional support, and advocacy to ensure clients’ rights and access to essential community services.
Address risks and needs through targeted interventions.
- Documentation & Reporting
Maintain accurate and timely records of all case-related activities and progress.
- Research & Evaluation
Keep abreast of current research, best practices, and trends in youth and family services.
Participate in and support research and evaluation projects to improve service delivery.
- Professional Development & Supervision
Attend and present cases at internal reviews, case conceptualisation meetings, and clinical supervision sessions.
Mentor and supervise interns and/or students on placement.
- Programme & Project Development
Conceptualise, develop, and implement new initiatives aligned with the Division’s objectives.
TECHNICAL COMPETENCIES
- Strong knowledge of specialised theories, assessment tools, and intervention techniques.
- Ability to conceptualise and formulate relevant, realistic, and achievable case plans.
- Skilled in engaging stakeholders to develop collaborative solutions in community work.

PREFERRED QUALIFICATIONS & ATTRIBUTES
- Degree in Social Work or equivalent.
- Minimum 2 years of experience in the social service sector or a similar setting.
- Solid knowledge and experience in social work interventions, with an interest in developing clinical skills.
- Genuine passion for working with youths.
