Job Summary:

Main Responsibilities:

• Develop individualized care plans tailored to each resident's needs.
• Execute care plans and monitor resident progress regularly.
• Coordinate care and communicate effectively with relevant stakeholders.

Requirements:

• Degree or Graduate Diploma in Social Work, Psychology, or Counseling.
• At least 1 year of relevant experience.
• Willingness to perform shift work and follow duty rosters.

Working Hours:

• Mon-Fri (Office hours), plus one "DO" shift per month and public holidays.
• Shifts vary between 8 am and 7 pm; DO shift is 8 am – 8 pm.
