Job summary

A leading healthcare institution in Singapore seeks a dedicated supervisory role that involves overseeing a team in community healthcare. Qualifications

  • Supervise team members in neighborhood outreach.

Responsibilities
  • Responsible for the supervision of team members in each neighbourhood

  • Conduct assessment and identify necessary intervention plan for client’s healthcare and social needs to support effective living in the community

  • Monitor client s care plans with healthcare professionals based on client s goals, assessed needs and services required, and monitor clients progress through regular case discussions with team members and volunteers

  • Coordinate care transition for clients in collaboration with the referring agency and other community partners involved

  • Provide education, information and referral based on client’s condition

  • Identify challenges and gaps relating to work processes and service delivery, and provide inputs or feedback to Community Manager to improve the quality of care

  • Any other duties assigned from time to time

Requirements
  • Degree in Social Work or healthcare related discipline with preferably at least 2 years of experience in a supervisory role in community or social service setting

  • Preferably a Registered Social Worker with Singapore Association of Social Workers (SASW) or registered with Singapore Nursing Board

  • Ability to speak local languages and dialects to interact and build rapport with elderly clients in the programme

  • Willingness to work flexible schedule as weekends and some evening hours commitment will be required

  • A team player with strong initiative and good communication skills, with proven track record of case management practice and skills
