Job description
  • Perform comprehensive and client-specific speech and language assessments during admission, regular care reviews, and discharge planning.
  • Utilize suitable outcome measures to monitor therapy effectiveness and inform timely clinical decisions.
  • Collaboratively develop personalized and SMART (Specific, Measurable, Achievable, Relevant, Time-bound) therapy goals with clients and key stakeholders.
  • Plan and deliver individualized, evidence-based speech and language interventions that prioritize client needs.
  • Accurately document assessments, therapy plans, goals, interventions, and outcome measures in line with clinical guidelines.
  • Communicate findings, treatment plans, progress, and discharge strategies clearly to clients and relevant parties.
  • Create discharge plans and strategies to support continued care, developed in partnership with the care team and client.
  • Promote successful integration into the community by working with relevant external agencies when necessary.
  • Recommend and prescribe assistive devices and communication aids tailored to each client’s needs.
  • Engage in multidisciplinary discussions to ensure holistic and coordinated client care.
  • Maintain accurate, timely documentation and records that reflect therapy provided and client outcomes.
  • Contribute to the planning, implementation, and evaluation of clinical programs and services.
  • Provide support in mentoring and supervising Therapy Assistants.
  • Actively participate in ongoing professional development activities.
  • Involve in quality improvement projects and/or clinical research initiatives.
  • Comply with audit standards and internal regulatory requirements.
  • Undertake other duties as directed by the reporting officer.

  • Bachelor’s Degree in Speech and Language Therapy and registered with AHPC
