Job description

Under the guidance of a Patient Navigator or Community Nurse, you will coordinate and navigate all aspects of patient discharge planning to ensure a seamless transition and continuity of care for patients from the hospital to the community, as well as coordinated care for frail residents in the community.

You will also be required to:

  • Establish strong working relationships with patients, their family, internal and external stakeholders and the community, and be the main point of contact for care coordination and navigation
  • Monitor and follow‑up on patients’ conditions regularly through phone calls or home visits to facilitate early detection of health issues and interventions, so as to prevent illness deterioration and hospital readmission
  • Responsible for educating patients their family on the self‑management of illness and importance of a healthy lifestyle, and for reinforcing individualised patient and caregiver education

  • GCE ‘N’/ ‘O’ / ’A’ level, NITEC
  • Candidates with relevant higher education certification and / or experience will be considered for senior position.
