A healthcare provider in Singapore is seeking a dedicated professional to facilitate family engagement at FamNex@OTH. In this role, you will connect families with essential health and social services, ensuring smooth operational processes. The ideal candidate has a diploma, strong communication skills, and experience in administrative roles, with preference for backgrounds in community care. This is a full-time position requiring commitment to a 5-day work week including Saturdays.
Family Nexus @ Our Tampines Hub (FamNex@OTH) is a one‐stop wellness hub to integrate health and social services for families. It hosts self-help resources for families, consultation rooms for health and social services, as well as activity spaces for conducting group activities for children, couples and families.
You will be based at FamNex@OTH and play a key role in engaging families and caregivers residing in the eastern region and connect them to services, activities and resources to support their health and social care needs.
You will provide support to ensure smooth operations within FamNex@OTH. You will also support the implementation of FamNex@OTH, such as working closely with health and social care providers to ensure the preparation and smooth execution of services and activities. You will also assist in data collection, reporting of key performance indicators and coordinating and supporting other operational activities such as meetings and writing of reports. Additional duties may be assigned by your supervisor as required.
