Post-Diagnostic Support Community Outreach Team (CREST-PDS), aims to enable persons living with dementia seek early treatment nearer to their home and ensure that they are well supported in the community. CREST-PDS provides education, coordinates community resources and facilitates early planning and management of the condition. It includes psychosocial interventions with the care planning for both Clients and Caregivers.

RESPONSIBILITIES
  • Support in initial assessment of referral cases, follow-up with intervention plans, as per the prescribed care plan and under guidance from senior
  • Support in the monitoring of Clients' conditions and progress, and escalated to SAHP where necessary
  • In-charge of timely data collation and quarterly report submission
  • Provide administrative or logistical support to the team in running Client-related support and/or other CREST-PDS activities
  • Prepare the necessary resource material to be provided to Clients and caregivers.
QUALIFICATIONS
  • Higher NITEC in community care or nursing, or other fields related to healthcare and community care
  • Has around 8 years of healthcare/community care experience
  • IT literate
OTHER INFORMATION
  • Good communication and social skills to work with different people.
  • Pleasant personality.
  • Team player, yet independent to carry out most functions.
  • Strong interest in empowering and preserving the dignity of people at-risk or with mental health conditions including dementia.
  • Able to converse in dialects preferred.
