Programme Associate @Day Care Centre (Community Nursing) #HAL

Location @ Day Care Centre (can choose): Chai Chee / Admiratly / Bukit Batok / Jurong Central

Working Hours: Mon-Fri, 8.30am - 6pm

Job Scope:

• Facilitate various multi-modality group activities for persons with dementia as assigned by the programme executive; including exercise sessions, meal preparation groups, cognitive games, music and art-related activities.
• Assist in activity planning and preparation to ensure the smooth execution of the group activities
• Manage clients with different mental, and physical conditions and behaviour of concerns relating to dementia and assist in the monitoring of client's health status
• Assist in client's care including providing necessary assistance for transfers, toileting and functional mobility
• Assist in administering assessments to clients and caregivers, perform data collection
• Perform timely documentation for the client post-session
• Assist to provide timely updates to family/NOK of client's engagement
• Support staff or volunteers relating to assist with running the programme
• Assist in inducting new community care associates and provide necessary guidance
• Assist in the procurement of activity materials, with approval sought from RO/Programme Executive
• Perform general housekeeping to the programme premise
