(Non Clinical) Locum Medical Social Worker (Must have Hospital Experiences) - $30 - 40/hr #HJC

Key Duties and Responsibilities

Community Care Medical Social Workers (MSW) are responsible for the assessment and management of patients referred for social work services by the Community Care Team and Community Health Posts. They also participate in multi-disciplinary meetings as needed to provide consultative services and identify patients requiring MSW input.

Clinical Duties

• Casework/Psychosocial Care
  • Provide end-to-end case management support and appropriate level of care through assessment, care planning, interventions and evaluation of care, including complex, specialized and high risk cases.
  • Staff is able to provide professional advice or review complex cases carried out by MDT.
  • Advocate for the wellbeing of patients and beneficiaries.
• Group Work
  • Able to plan, lead and facilitate group work sessions independently and build social support systems for target groups.
  • Reach out to individuals and/or target groups who may benefit from group work.
  • Able to support the lead in the planning of group work evaluation.
• Community Work
  • Assist in planning and implementation of existing and/or new programmes which enhance psychosocial well-being of the community.
  • Participate in programme and activities evaluation.
  • Deliver public education talks with guidance.
  • Network with targeted community partner.
• Programme/Project Development and Implementation
  • Assist in the development and implementation of programmes.
  • Lead department programme/project to improve the quality of care and services of patients and caregivers.
  • Participate in inter-department programme/project to improve the quality of care and services of patients and caregivers.

Education / Clinical Supervision

• Participate in developmental programmes, learning opportunities and supervision for personal and professional growth.
• Provide guidance and training to interns, support staff and junior MSWs.
• Provide clinical supervision to fieldwork interns and support staff.
• Assist to organise, coordinate and provide continuing education to department and inter-department colleagues and community partners.
• Assist to develop resource materials.
• Assist to conduct talks, professional workshops and seminars.
