A local healthcare organization in Singapore is seeking a Staff Nurse to provide home-based nursing services to frail elders. The successful candidate will assess client needs, collaborate with healthcare professionals, and implement care plans. Candidates should be registered nurses with relevant qualifications and at least 2 years of experience in eldercare. Strong communication and multitasking skills are essential.

Qualifications

  • State registered with the Singapore Nursing Board and possess a valid Practising Certificate.
  • Preferably 2-3 years working experience in the eldercare sector.

Job description

This job role is being offered by Tsao Foundation.

The Staff Nurse collaborates closely with the doctor and social worker to provide home-based nursing service to the frail and home bound elders to meet the Foundation’s mission and operational targets, in accordance with the standards of care and service protocols established by the Foundation and the community. They are open to hire Seniors who are aged 50 and above. Please note that this position does not offer work visa sponsorship.

Key Responsibilities
  • Conduct home visits and perform comprehensive nursing assessments regularly to homebound frail elders
  • Able to triage client conditions and respond in a timely manner to address the issue
  • Be part of the interdisciplinary team to develop and implement client’s care plan
  • Explain care goals and care plans to client and caregivers to gain active participation from them
  • Evaluate outcome interventions and revise care plans as needed
  • Manage and monitor client’s medical and nursing needs as required
  • Maintain documentation of client’s progress and outcomes of care provision
  • Assist Doctor in charge and perform nursing procedures as needed
  • Assess and provide appropriate education and training for clients, caregivers and team members
  • Carry out necessary administrative duties in line with internal and external agency guidelines and SOPs
Key Requirements
  • Professional Certificate/Specialist Diploma/Advanced Diploma in Nursing, preferably with training in gerontological nursing.
  • State registered with the Singapore Nursing Board (SNB) and possess a valid Practising Certificate in Singapore
  • Preferably 2-3 years working experience in the social services or eldercare sector.
  • Extensive clinical knowledge and skills
  • Penchant for working with the elderly and caregivers
  • Flexible, energetic and able to multi-task
  • Team player with competent communication and interpersonal skills
  • Bilingual in 2 or more local dialects as you will be required to communicate with elderly person within the community.

Working Location: West Region

Salary is indicative and commensurate with qualification and experience.
