Overview
iKare provides holistic engagement programs to clients at their homes. Currently, iKare offers ‘Cognitive Intervention Programme’, a home care program focused on strengthening one’s cognitive and motor functions, and ‘Spark’, a more comprehensive home care program that supports cognitive, physical, and emotional well-being.
The Home Engagement Nurse is vital in providing personalised care and therapy to clients in their homes as a planner and facilitator of our programs. This role combines nursing, care planning, and the facilitation of a home engagement program to support clients\' well-being.
Key Responsibilities
- Perform home assessment using iKare-developed assessment methods and tools. (Necessary training and tools are to be provided by iKare.)
- Design tailored care plan for individual clients based on the assessment and following iKare’s care planning formula.
- Facilitate engagement in adherence to the care plan.
- Perform Nursing Assessments, act as a general wellness monitor for the client, and communicate any wellness concerns to the client, primary caregiver, or doctor if necessary.
- Provide other home care services, including Activities of Daily Living (ADLs)
- Attend company-provided training and other required job-related training programs dutifully and maintain the necessary level of competency.
- Attend to inquiries and contact potential clients to identify their needs and provide information about iKare\'s services.
- Participate and assist in company events, fairs, education talks, and training workshops.
Qualifications
- Enrolled or Registered Nurse with a minimum of 3 years of relevant nursing experience in general or private hospitals.
- Diploma in Nursing or similar, plus licenses to practice nursing in Singapore.
- Experienced in home nursing and/or Dementia Care is preferred but not mandatory.
- Excellent observational, leadership and problem-solving skills.
- Ability to multi-task, work independently, and be proactive in making iKare a better workplace.
- Excellent communication, strong interpersonal skills.
- Ability to write and communicate in English
- Computer literacy.
- Passionate about care at-home, 1-to-1 care.
Preferred but NOT Required
- A person with experience or expertise in mental health care.
- A person with experience or expertise in elder care.
