RESPONSIBILITIES

A Family Coach plays a key role in supporting persons with intellectual disabilities and their families through important life transitions, such as moving from school to community or between residential and community care. Based in MINDS Regional or Community Hubs, you will work closely with a multi‑disciplinary team to develop and review Family Support Plans, while engaging caregivers to set goals, provide resources, and monitor progress. Using a coaching and partnership approach, you will help families build resilience and ensure that every transition is not only smooth but also sustainable.

Individual & Family Support
  • Coordinate and conduct 3‑5 yearly family meetings to review person‑centred goals and facilitate inclusive discussions where everyone's voice, including the individual with disabilities, is heard.
  • Support families/caregivers and clients through transitions, home visits, and community assessments, empowering caregivers with skills for daily living and community integration.
  • Collaborate with Transition Support Officers and Training Officers to align goals, strategies, and skill transference from structured settings to real‑life environments.
  • Provide generic case management and guide families through transitions such as school‑to‑work or residential‑to‑community care.
Collaboration & Planning
  • Provide information and referrals, coordinate services and resources required, and work closely with psychologists, social workers, therapists, and other professionals to implement Individual Transition Plans (ITPs).
  • Conduct transition planning meetings relevant to future care planning and maintain the Future Ready Toolkit; conduct training and consultation services to raise understanding and awareness of components of future care planning in the area of legal, financial, health and social support.
  • Liaise with community partners, employers, and stakeholders to build inclusive support networks.
Monitoring & Documentation
  • Assess readiness for transition using tools like CAF‑R.
  • Maintain accurate records of progress, challenges, and adaptations.
  • Provide timely updates to families and stakeholders.
  • Consolidate good and best person‑centred practices, frameworks, and knowledge and experience to build a resource bank of best practices and useful tools in collaborative practice, to inform practitioners for replication success, and / or pitfalls to avoid; include shared progress trackers and communication logs to stay aligned.
Review and evaluate the Family Plans in alignment to IDP
  • Conduct re‑assessment of family plans as needed, to track progress, i.e. pre and post intervention assessment.
  • Work with stakeholders and multi‑disciplinary team to revise / adapt IDP goals and strategies if outcome is not met, or to bring clients to a higher level of functioning.
  • Document and keep records of outcomes of clients' IDP and produce a consolidated report/dashboard on overall clients' IDP outcomes.
QUALIFICATIONS
  • Minimum Diploma in Social Work, Psychology, Special Education or other relevant fields with at least 2 years working experience.
OTHER INFORMATION
  • Excellent written and verbal communication skills.
  • Experience working with individuals with intellectual disabilities or special needs.
  • Willing to travel to community and work sites.
  • Ability to speak an additional local language apart from English.
  • Strong interpersonal, organisational, time management, and problem‑solving skills; self‑motivated and adaptable, with a passion for supporting persons with intellectual disabilities, advocating for families, and promoting inclusion.
