Job summary

Job description

You will be part of the team to provide high quality patient care and education in the following settings.

Inpatient
  • Gather patient’s DM and other medical history, admission issues, relevant medications and labs
  • Assess patient/caregiver on their social setup and diabetes self-management
  • Present the case to the IP DM Registrar/consultant, suggest appropriate inpatient treatment plans to optimize DM control according to guide.
  • Provide advanced diabetes care education to patient/caregiver
  • Recommend appropriate discharge plans including treatment regimens and post discharge follow-up
  • Provide post-discharge phone consultation service for complex DM patients who need insulin titration
  • Coordinates with respective internal and external care provider in coordinating continuity of care for patient
Specialist Outpatient Clinic (Endocrine)
  • Provides diabetes counselling and education to patient and their family
  • Performs close follow-up on patients with suboptimal glycemic control via phone calls, emails, SMS, and other communication applications.
  • Suggest appropriate insulin titration in consultation with endocrinologist/ APN or according to guide
  • Actively contribute to the multi-disciplinary team’s plan and care of the patients
  • Conducts structured caregiver training programs as needed
Professional Development and Leadership
  • Participates in evidence-based practices and nursing research activities
  • Leads and participates in quality management audit and activities
  • Conduct staff training in the relevant specialty
  • Acts as a resource person for consultation in the relevant specialty
Requirements
  • DNE or Resident Nurse with experience, keen interest in diabetes management and patient education
  • At least 5 years of experience as a Staff Nurse in a clinical setting
