Job summary

An established social service organization in Singapore is seeking social workers to conduct biopsychosocial assessments, provide counselling, and support rehabilitation efforts. The ideal candidate will be involved in community outreach and work collaboratively with various professionals to ensure comprehensive care. Responsibilities

  • Conduct biopsychosocial assessments and formulate Individual Service Plans (ISPs).
  • Provide counselling to residents and families, including crisis intervention when required.
  • Prepare timely case notes, progress reports, and submissions in line with organisational and MSF requirements.
  • Provide Information & Referral (I&R) services to link residents and families with appropriate community resources.
  • Participate in the planning, implementation, and evaluation of programmes and activities that support rehabilitation and reintegration.
  • Assist in organising social outings, family sessions, and community-based programmes.
  • Support outreach initiatives to raise awareness of services available.
  • Work collaboratively with counsellors, psychologists, and other professionals to ensure holistic care for residents.
  • Advocate for residents’ needs and rights during case conferences and multidisciplinary discussions.
  • Engage families to strengthen support systems.
  • Participate in training, supervision, and reflective practice sessions.
  • Share knowledge, best practices, and learning with team members to improve service quality.
  • Uphold professional ethics, confidentiality, and service standards in all interactions.
  • Undertake additional tasks as assigned by the Senior Social Worker.
