Job description

YouthReach provides psychosocial support and recovery programmes for youths (aged 12 to 21)

Emotional, psychological and psychiatric issues in the community

Staff work with youths and their families to empower them to achieve their goals and live in a safe, caring, and inclusive community

Objective

To ensure continuous quality of the programme and service delivery to youth & families. To work with the youth, family & community partners to enhance family strengths and resources in order to minimize risk and improve functioning.

Client Management
  • Provide individualized, comprehensive, community-based services
  • Implement intensive case management service model in working with youth and their family
  • Conduct intake assessment for youth referred to the centre
  • Assist family in crisis preparation and safety planning
  • Maintaining of progress notes, reports on youths' progress and review of goals and performance at 6 monthly intervals and maintain audit ready files
  • Promote the goal setting and achievement of goals by the client and their respective family members
  • Work closely with families, schools and hospitals and job places to support reintegration of client into community setting
  • Work with families to improve knowledge of mental illness and ability to handle crisis/relapse
  • Provide counselling and support to youth in coping with emotional, or psychological difficulties
  • Manage, supervise and motivate the participants of the programme
  • Participate in internal as well as external case conference with community agencies, i.e. hospitals, schools and social service agencies as needed
  • Work as part of a multi-disciplinary team within organisation and also with external partners and community agencies as needed
  • Provide after office hours support to youth, and caregivers on support line (on rotational basis)
Administrative & Support Services
  • Conduct external talks for SAMH annually
  • Conduct outreach to schools and community
  • Act as a referral point for youth and their families to tap on suitable services in the community
  • Implement and monitor various programmes to meet the needs of the youths and their family served
  • Help facilitate public forums and outreach events
Other Duties
  • To work on adhoc project & community outreach programmes and events ie World Mental Health Day, community engagement events, etc.
  • To carry out any other duties assigned by the Agency/Executive Director
