A community service organization in Singapore seeks an individual to provide basic case management support while enhancing the well-being of clients. The role involves offering emotional support, assisting clients and caregivers, and organizing community outreach initiatives. Ideal candidates should possess a diploma or bachelor's degree, have good organizational and interpersonal skills, and 2–3 years relevant experience preferred, although those without experience may apply. A passion for working with people and knowledge of social services is advantageous.
The incumbent provides basic case management support under the guidance of the supervisor to enhance the well‑being of clients and help them age in place within the community. The role includes supporting clients and caregivers, assisting in group activities and community outreach, and collaborating with stakeholders to ensure coordinated care.
Provide basic emotional support and regular monitoring to clients living in the community.
Assist clients and caregivers with information on assistance schemes, care options, and connections to community resources.
Support volunteers in befriending and client engagement initiatives.
Collaborate with stakeholders to ensure holistic and coordinated care for clients.
Participate in case discussions, reviews, and supervision sessions within the Centre and with external stakeholders.
Assist in planning, coordinating, and facilitating Cognitive Stimulation Activities and group programmes at the Centre.
Maintain timely and accurate case progress notes, home visit logs, and documentation in compliance with regulatory standards. Assist in preparing reports for funding agencies.
Support outreach initiatives such as community mental health screenings, health talks, and public awareness events.
Participate in developmental programmes and supervision to enhance professional skills and knowledge.
Perform other duties assigned by the Reporting Officer.
Diploma / Bachelor’s Degree in any discipline
2 - 3 years relevant experience, candidates without experience are welcome to apply
Have a passion to work with people; knowledge of social services is preferred
Have good organizational skills and administrative skills, is team‑oriented with excellent interpersonal and communication skills; be resourceful and collaborative;
Ability to converse in various languages and dialects is an advantage
