Community Support: CCMS (Community Case Management Services) | CREST (Community Resource, Engagement and Support Team) | Henderson Home

Job Description

He or she will be required to provide supportive services to seniors of the Henderson Home/Staycay and CCMS clients. The role focuses on (i) maintaining a safe and clean home environment for residents, (ii) engaging residents and raise their concerns to social workers or Head of Home, (iii) partnering colleagues in providing holistic care for residents.

You will need to:

  • Monitor the well‑being of seniors, ensuring their safety, comfort and care needs are met
  • Assist seniors with monitoring their own health e.g. blood pressure and glucose monitoring
  • Maintain a clean and safe environment for residents
  • Support residents with exercises and medication adherence
  • Coordinate cases of seniors with social workers
  • Assisting in new resident admissions
  • Organise visits for residents medical check‑ups and accompanying them to medical appointments
  • Maintain the residents movement logs for essential needs such as doctor appointment
  • Facilitate seniors access to essential social and healthcare services, eg meal services, home care, home help, hospital admissions, medical appointments and counselling service etc.
  • Track visitors activity by the visitor log book
  • Support outreach initiatives at Henderson Home/Staycay
  • Facilitate group activities to enhance seniors social interaction and mental well‑being
  • Provide emotional support to seniors facing challenges
  • Mediate conflicts between seniors when necessary
  • Manage personnel, finance and operations of the Sheltered Home/Staycay
  • Document care activities in System
  • Submit reports as required by funders
  • Participate in team meetings and training
  • Conduct home visits and outreach activities to the community
  • Other ad‑hoc assignments at the discretion of program lead/ Department head
Qualifications
  • Diploma/Certificate preferably in office or business administration, social work/psychology/sociology/gerontology/counselling/nursing or its equivalence
  • Good communication and interpersonal skills (oral and written)
  • Passion in community services and service oriented
  • Good judgement and decision making skills
  • Good in problem solving
  • Good record keeping abilities and attention for details
  • Meticulous and independent
