Responsibilities:
Clinical Activities
- Overseeing daily ward workflow and patient care coordination
- Supporting complex clinical decision-making and escalation processes
- Conducting regular ward rounds to monitor care standards and patient safety
- Ensuring adherence to nursing protocols, care pathways, and clinical SOPs
- Supporting high-acuity or high-risk patient management (falls risk, pressure injuries, MDRO, tracheostomy care, wound management, etc.)
- Ensuring appropriate clinical handovers and continuity of care across shifts
- Providing on-site clinical support during emergencies and deteriorating patient situations
- Participate actively in clinical improvement projects.
Staff Supervision & Development
- Supervising daily nursing activities and task delegation
- Providing coaching, mentorship and real-time feedback to staff
- Supporting orientation and onboarding of new hires and locum staff
- Facilitating clinical competency assessments and skills validation
- Identifying staff training needs and coordinating continuous education activities
- Supporting performance appraisals, probation reviews and development planning
- Reinforcing professional conduct, workplace discipline and ethical practice
Quality & Patient Safety Initiatives
- Participating in quality improvement projects aligned with organisational KPIs
- Leading or supporting clinical audits (medication safety, falls prevention, infection control, documentation compliance)
- Monitoring incident trends and supporting corrective action planning
- Facilitating Root Cause Analysis (RCA) participation when required
- Promoting safety culture and reporting compliance among staff
- Implementing evidence-based practices to improve patient outcomes
- Tracking improvement outcomes and reporting progress to management
- Collaborate with nursing, allied health professionals, and support care teams to orchestrate improvement endeavours and continuous improvement projects
Resource & Manpower Coordination
- Assists with duty roster planning and overseeing shift allocation when required
- Ensuring safe staffing levels and skill mix across shifts
- Coordinating manpower coverage during sick leave, emergencies and peak workload periods
- Monitoring staff attendance, punctuality overtime
- Ensure all leave applications are reviewed and approved in a timely manner.
- Practicing stewardship when supporting deployment of locum or agency staff when required
- Ensuring fair workload distribution across nursing teams
- Supporting equipment availability and ward supply readiness
Regulatory Compliance & Audits
- Supporting compliance with Hospital to Home (HHOM), Ministry of Health (MOH) licensing conditions and SNB professional standards
- Ensuring staff adherence to organisational policies and SOPs
- Participating in internal and external audits and inspections
- Supporting documentation readiness for regulatory reviews
- Monitoring compliance with infection prevention and control standards
- Identifying gaps and implementing corrective actions
- Supporting accreditation and licensing preparation exercises
- Collect data required to meet audit and performance management targets in a timely manner.
Administrative Reporting & Documentation
- Supporting preparation of nursing operational reports and dashboards
- Monitoring documentation compliance in patient records and clinical systems
- Supporting incident reporting, audit submissions and regulatory documentation
- Maintaining staff training and competency records
- Supporting inventory tracking and equipment documentation
- Coordinating administrative communications between nursing, HR, operations and management teams
Systems and Administrative Support
- Assist in realignment of clinical, nursing and personal care staff within the continuum care system.
- Answer queries from internal and external parties in relation to clinical and nursing matters.
- Provide administrative support in clinical quality improvement projects and programmes.
- Comply with the organisation approved standards, procedures, protocols and policies.
- To adhere to the Personal Data Protection Act (PDPA) relating to clients and staff.
- Perform other related duties as assigned by Reporting Officer / Head of Nursing.
Requirements:
- Possess a Registered Nursing Certificate
- Registered with Singapore Nursing Board
- Minimally possess a Diploma in Nursing and Advanced Diploma in a speciality (preferably Medical, Geriatrics, Palliative
- Possess a professional demeanour and conduct themselves with integrity and respect towards all patients, colleagues and other parties
- Demonstrate patience and approachability while assisting patients and handling challenging situations effectively
- Demonstrate effective communication skills, both verbally and in writing, to interact with patients, colleagues and other parties
- Display adaptability and flexibility to work efficiently in diverse settings with changing demands and schedules
- Familiarity with regulatory guidelines and compliance standards.
- Possess good interpersonal and communication skills, with a positive work attitude and strong commitment towards service excellence
- Be a mature and objective lead, possess leadership qualities and positive interpersonal skills.
- Possess up-to-date Basic Cardia Life Support (BCLS) + Automated External Defibrillator (AED) Certificate.
- Must be able to work shift, occasional weekends and night shifts and public holidays monthly.
- Willing to go on rotational on-call roster weekly.
