Bank Hospice at Home Healthcare Assistant

Beaumond House Hospice Care delivers personalised, high-quality end of life care across hospice, home, and community settings. Patients and families remain at the centre of everything we do. All staff are expected to uphold dignity, safety, confidentiality, and the highest standards of physical, psychological, and spiritual care.

This role sits within the End-of-Life Care Together Alliance and operates across organisational boundaries to deliver safe, coordinated, and person-centred care for people in North Nottinghamshire.

This role will be based at Beaumond House Hospice Care, Newark and community settings across North Nottinghamshire.

Main duties of the job

Role Purpose

To provide individual high-quality care for patients in their own homes, including appropriate support for families, in line with hospice policy and agreed care plans. Care may include supporting patients while symptoms are assessed and brought under control, caring for the dying, supportive psychosocial and spiritual care in line with any agreed plan, enabling patients independence and supporting discharge home.

Working under the direction of a registered nurse and as part of the Beaumond House team and the End-of-Life Care Together Integrated Hub you will help maintain the standard of excellence, so the philosophy and values of Beaumond House Hospice Care are met.

Beaumond House Hospice Care is a registered charity. We provide four In-patient beds, Day Therapy, and Hospice at Home Care. Hospice at Home supports people with palliative and end of life care needs, often in the last weeks and months of life. Our nurses and health care assistants provide assessment, personal care, emotional support, advice, and advance care planning in the persons preferred place of care.

We are contactable 24 hours a day, 7 days per week, to provide a friendly ear, general information or to signpost patients, relatives and carers to appropriate resources and services.

Job responsibilities

Main Duties

Work under the direct supervision of the RN and in line with the patients care plan, providing care to meet fundamental health care needs. This includes assisting patients with toileting, personal care and supporting patients to eat and drink as independently as possible whilst maintaining privacy and dignity at all times.

Following induction and assessment of competence, the post holder undertakes delegated visits independently in line with written care plans, with clear escalation routes to the Registered Nurse or Hospice at Home Lead.

Medication support is provided following completion of medicines support training and competency assessment, in line with hospice medicines policy. This includes prompting, assisting, and where authorised, administering prescribed medicines as recorded on the medication administration record. The post holder does not make clinical decisions regarding medicines and escalates concerns immediately to the Registered Nurse.

Following completion of food hygiene and food safety training, the post holder prepares and serves simple meals, snacks, and drinks in accordance with hospice policies on food hygiene, allergens, storage, and patient preferences.

Provide care for patients and families following death. Assist in care after death, respecting spiritual, cultural and religious needs.

Following training and formal competency assessment, the post holder carries out verification of expected death in the community for patients known to the service, in line with Beaumond House policy and local guidance.

Complete all required documentation and communicate the verification of expected death promptly to the RN, GP, the Integrated Hub and any other relevant professionals, and escalate to the RN or on call clinician if you have any concerns or if the criteria for verification of expected death are not met.

Support families with practical next steps as appropriate, including liaising with funeral directors in line with hospice guidance.

Ensure safe custody of the patients property and follow hospice procedures for recording and handing over valuables.

Responsibility is taken for patient safety, with care delivered only within the scope of training and competence and in accordance with verbal direction from the Registered Nurse or written care plans. This includes care for patients with reduced capacity, cognitive impairment, or sensory loss.

Care is delivered in a non-judgemental manner, treating each patient as an individual and respecting choices, wishes, customs, values, and beliefs.

Establish the preferred method of communication for the patient and their family and communicate effectively and appropriately with patients from diverse backgrounds, including different cultures, and with patients who have sensory loss, communication difficulties or challenging behaviour.

Queries from patients and families are managed sensitively. Concerns are escalated appropriately, and confidentiality is maintained at all times.

Attend team meetings and other touch points such as one to ones and after death reviews.

Establish and maintain good working relationships with other staff members and volunteers.

Communicate appropriately with all members of the multidisciplinary team, other colleagues, patients, relatives and loved ones, always displaying a friendly and approachable manner.

Assist in the orientation and induction of new staff, students, bank and agency staff, including use of equipment and basic care skills.

Attend relevant study days and courses, including mandatory training, medicines support, verification of expected death and food hygiene, to gain and maintain the knowledge and skills set out in the knowledge and competency framework.

Infection prevention and control principles are always followed.

Contribution is made to maintaining a safe environment through risk identification, incident reporting, and safe management of equipment, including disposal of sharps and clinical waste.

General Duties

Work collaboratively and cooperatively with your Beaumond House colleagues and End of Life Care Together Alliance partners.

Ability to work within defined scope of practice, follow care plans, and escalate concerns appropriately to a Registered Nurse.

Respect confidentiality in all areas of Beaumond House.

Compliance with health and safety legislation and hospice policies is required, including reporting of incidents and near misses.

Equipment issued safely and appropriately in line with training.
