Job description

The average daily emergency "take" in Adult Medicine is 35 patients (+25 for Care of the Elderly) and the average length of stay of < 4-5 days. Consultant post-take ward rounds are carried out continuously in the EQ (Emergency Quadrant) There are over 6,000 emergency admissions each year to Adult Medicine, with a further 4,000 annual admissions to Care of the Elderly.

The Division of Emergency and Integrated Medicine has twice-weekly sessions of postgraduate and audit activity, including a weekly Physicians' Grand Round. There is also a weekly lunchtime Clinical Conference. The Division has a regular teaching commitment with resident students from Cardiff, Manchester and Liverpool, with the majority of appointed from these Universities. A core curriculum is taught to Foundation Year Ones and SHOs/FY2's, and there are also regular MRCP tutorials. There is a dedicated Clinical Governance Unit and Audit Office with permanent staff.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

The candidate will be responsible, both professionally and managerially, to Dr H Nair clinical lead for acute medicine. You will also be supervised by a team of consultant's hrs within acute medicine

They will:

a. Provide support to Consultant and Associate Specialist, in managing the department in-patients and out-patients (ambulatory patients)

b. Provide leadership and education to the junior doctors on the team.

  1. Provide On Call as a Middle Grade.
  2. Contribute to local postgraduate activity.
  3. Participate in Clinical Audit and CPD, which is fully supported by the Health Board.
  4. Participate in research projects as necessary.

Shared Office accommodation and access to intranet/internet will be provided.

  • MBBS or equivalent medical qualification
  • MRCP (UK) Part 1 or equivalent at time of application or work toward obtaining Part 1 within 6 months of being appointed.

  • Intercalated BSc or equivalent
  • Higher degrees including MSc, PhD or MD (where research thesis not part of first medical degree
Eligibility
Essential
  • Eligible for full registration with the GMC at time of appointment and hold a current license to practice
  • Evidence of achievement of Foundation competencies from a UKFPO affiliated Foundation Programme of equivalent by time of appointment in line with GMC standards/Good Medical Practice including: - ?Make the care of your patient your first concern ?Protect and promote the health of patients and of the public ?Provide a good standard of practice and care ?Treat patients as individuals and respect their dignity ?Work in partnership with patients ?Be honest and open and act with integrity
  • Evidence of achievement of CT/ST1 competences in medicine at time of application and CT/ST2 competences in medicine (as defined by the curricula relating to Core Medical training) supported by evidence from work-based assessments of clinical performance (DOPS, Mini-CEX, CBD, ACAT) and Multi-Source Feedback or equivalent, ARCP or equivalent
  • Eligibility to work in the UK
  • Not previously relinquished, released or removed from a training programme in the specialty except under exceptional circumstances

Language Skills

  • All applicants to have demonstrable skills in written and spoken English adequate to enable effective communication about medical topics with patients and colleagues demonstrated by one of the following: - ?That applicants have undertaken undergraduate medical training in English; or ?Have achieved the following scores in the academic International English Language Testing System in a single sitting within 24 months at time of application - Overall 7, Speaking 7, Listening 7, Reading 7, Writing 7. If applicants believe they have adequate communication skills but do not fit into one of these examples they must provide supporting evidence
Clinical skills
Essential
  • Appropriate knowledge base and ability to apply sound clinical judgement to problems
  • Awareness of the basics of managing cardiovascular disease
  • Able to demonstrate proficiency in a range of medial procedures (as defined by the CMT curriculum) as an indication of manual dexterity and hand-eye coordination
  • Evidence of competence in management of medical emergencies and in-patients through continuous work-based assessments, portfolio evidence, including log book documentation if applicable
  • Evidence of competence to work without direct supervision where appropriate
  • Demonstrate current ALS certification or equivalent
Desirable
  • Demonstrates awareness of the basics of managing general medical emergencies
  • Evidence of some competences in the specialty as defined by the relevant curricula
  • Evidence of skills in the management of acute medical emergencies (e.g. ALERT, IMPACT certification)
  • Evidence of ST3 equivalent procedural skills relevant to medical patients (clinical independence in central venous cannulation, chest drain insertion, joint aspiration, DC Cardioversion, abdominal paracentesis)

  • Demonstrates understanding of research, including awareness of ethical issues
  • Demonstrates understanding of the basic principles of audit, clinical risk management, evidence based practice, patient safety and clinical quality improvement initiatives
