Qualifications

Responsibilities

• Lead coordination of adult patient discharges.
• Participate in MDT huddles and board rounds.
• Develop discharge plans within 24 hours of admission.
• Manage and support ward teams in discharge management.
• Liaise with patients, families, and carers during discharge planning.
