Job description
Overview

Are you a paramedic or a nurse looking for an opportunity to take your career in an exciting new direction? Do you have the ability to prioritise your workload effectively and efficiently, and experience in clinical triage and delivering care in community settings? If you answered yes, we’ve got the perfect role for you!

We are looking for proactive clinicians to join our urgent response service, providing urgent care to unwell patients in their own home or care homes within 2 hours of a healthcare professional referral. The multi-disciplinary team consists of professionals from different clinical settings and works across Medway supporting primary care, community services and the ambulance trust to keep patients safe and well at home. The successful applicant will have experience of clinical triage and delivering care in community settings.

Responsibilities
  • To work as part of the multi-disciplinary urgent response service to support unwell patients in domiciliary and care homes.
  • The service operates from 07:00–19:00, 7 days a week; post holders are required to work 12‑hour shifts.
  • Rotate between clinical triage function and face-to-face patient consultations.
  • Undertake clinical triage for health care professional referrals into the urgent response service.
  • Undertake face-to-face assessment, diagnosis and treatment for community patients presenting as unwell to prevent hospital admission where appropriate and keep patients safe within their own homes.
  • Work as a member of a multidisciplinary team providing clinical care for patients within research and evidence-based guidelines (team includes advanced clinical practitioners, paramedics, phlebotomists, therapists, care managers and social care providers).
  • Support clinical leads in managing cost-effective and efficient practice and actively seek to prevent hospital admissions where appropriate.

Knowledge, training and experience
  • Hold current registration as a Paramedic (HCPC) or Nurse (NMC).
  • Evidence of clinical triage experience.
  • Qualification in Minor Illness Management, Enhanced Clinical Assessment or willingness to undertake.
  • Understanding of community care settings and integrated working.
  • Regular training and ongoing professional development; IV therapy and cannulation training or willingness to undertake.
  • Mentor preparation/ENB 998 or equivalent; good communication and interpersonal skills; awareness of child and adult protection issues.
  • Knowledge of Clinical Governance, risk management and IT skills (Windows, Internet, Outlook).
  • Ability to manage time, conduct research, be assertive and resilient; familiarity with National Service Frameworks and Minor Illness Management pathways.
Analytical and judgement skills
  • Ability to prioritise and respond appropriately to patient referrals.
  • Assess patients with a range of conditions, diagnose, advise on treatment, and refer to colleagues as appropriate.
  • Proactive risk management and valuing patient, family and colleague feedback.
  • Follow approved pathways while using own judgement to refer or seek guidance as needed.
Planning and organisational skills
  • Initiate and organise services to ensure appropriate care at home, liaising with other specialist teams as needed.
  • Report patient conditions to senior staff and ensure prescribed treatments are carried out.
  • Engage in self-development, clinical supervision, and objective setting; manage time to meet patient demands; adapt to changing service needs.
  • Assess, plan, implement and evaluate care programmes in community settings, including care homes.
Physical, HR and information responsibilities
  • Perform physical examinations and a range of clinical skills, including wound care, IV therapy and cannulation as appropriate.
  • Deliver research-based practice and ensure care is safe and effective; prescribe treatments where appropriate via PDs or independent prescribing.
  • Maintain records, safeguard confidentiality, and submit accurate data for caseload management.
  • Supervise and train students and junior staff; participate in appraisal and clinical supervision; attend staff meetings.
  • Participate in clinical audit, governance activities and data collection to measure outcomes.
  • Responsible for safe use of equipment and adherence to procurement procedures; identify opportunities to improve patient care standards.
Policy and service development
  • Participate in clinical audit, governance activities and data collection for service development.
  • Contribute to integrated/interprofessional patient care and the achievement of service objectives.
Freedom to act and working conditions
  • Work within organisational policies and guidelines; act professionally and ethically with a high standard of clinical care.
  • Be aware of physical, mental and emotional effort and working conditions as part of the role.
