A healthcare organization in Faversham is seeking Nurse Practitioners to join their supportive team. Responsibilities include providing consultations for minor injuries and illnesses, conducting assessments, and ensuring adherence to clinical governance. Candidates should have NMC/HCPC registration and excellent communication skills. An independent prescriber qualification is beneficial. The position requires flexibility, including weekends and bank holidays, with a competitive salary and additional premiums for unsocial hours.
We are looking for upto full time Nurse Practitioners, Paramedic Practitioners to work with ourexpanding friendly and supportive team at Faversham Medical Group and UrgentTreatment Centre.
An independentprescribing qualification would be beneficial but is not essential.
This position requiresworking some Bank Holidays, regular weekends, evenings and other cover whenneeded as our Urgent Treatment Centre is open 7 days a week 8am to 8pm.
We are based in aHealth Centre with newly renovated consulting rooms and X-ray facilitieson-site. You will be joining a team of urgent care practitioners, GPs, ourpractice nurses and health care assistants.
Staff working out ofhours will receive an unsocial hours addition. This is paid at additional 30%rate for working on a Saturday and additional 60% rate for working on a Sundayor Bank Holiday, during contracted hours.
We also give aprescribing premium of £3,000 gross per annum to FTE UCPs that havesuccessfully passed the prescribing qualification and have become independentprescribers, payable over 12 months.This is pro-rata for part-time UCPs.
The post holder will be responsible for providing consultations for patients presenting withminor injuries, unscheduled and scheduled primary care conditions and minor illnesses. Theywill act as an autonomous clinician and take responsibility for the assessment, treatment anddischarge of patients.
Faversham Medical Group consists of 2 GP surgeries;Faversham Medical Practice and Newton Place Surgery. We also run an UrgentTreatment Centre based at Faversham Health Centre.

Job responsibilities and Accountabilities:
Assistthe Clinical Lead and Medical Director in implementing systems and processesthat will ensure excellent standards of care
Undertakeface to face assessment and treatment of patients who attend the UrgentTreatment Centre [UTC] and Faversham Medical Group, including home visits.
Provideassessment, treatment and diagnosis at point of first contact, by attending topatients in a variety of clinical or non-clinical settings according topatients needs including care planning.
Referpatients to an alternative care setting or treat and discharge as appropriate
Ensurethat professional standards are maintained in the provision of care within theguidance by the Department of Health and the NMC, HCPC or other relevantauthority.
Ensurethat the robust system in place for maintaining clinical governance withinFaversham Medical Group (FMG) is adhered to.
Implementthe monitoring plan designed by the Clinical Lead, Medical Director (MD) toevaluate standards of evidence based care and practice. Report results to the Clinical Lead andassist in developing action plans where necessary to improve standards andfeed-back the results to the Clinical Lead and MD
Networkwith external care providers to ensure continuity of care
Linkwith the Clinical Governance Team to ensure clinical policies and proceduresare current
Management:
Provideleadership and support to Receptionist/HCAs
Assistthe Clinical Lead to establish the mandatory training requirements of the HCAroles and identify training resources
Ensurean efficient and effective service delivery by actively supporting the ClinicalLead and other members of the team
Ensurerecords are kept on FMG activities and produce statistical data
Ensureaccurate learning and development records are maintained
Professional and Clinical:
Provideprofessional leadership for all Receptionists/HCAs providing a source ofadvice, consultation and support, to lead and motivate
Beconversant with the NMC or HCPC Code of Professional Conduct and any associatednational guidelines and abide by their guidance
Supportthe implementation of any audit findings
Communication and Key Working Relationships:
Thepost holder must be able to demonstrate excellent communication andinterpersonal skills at all times, and build and maintain good workingrelationships with all stakeholders including the following:

Administration and professional responsibilities:
Participate in the administrative and professional responsibilities ofthe practice team.
Ensure accurate and legible notes of all consultations and treatmentsare recorded in the patients notes.
Ensure the clinical computer system is kept up to date, with accuratedetails recorded and amended.
Ensure appropriate items of service claims are made accurately,reporting any problems to the practice administrator.
Ensure accurate completion of all necessary documentation associatedwith patient health care and registration with the practice.
Ensure collection and maintenance of statistical information requiredfor regular and ad hoc reports and audit.
Attend and participate in practice meetings as required.
Restocking and maintenance of clinical areas and consulting rooms.
Supplies and equipment:
Maintenance of equipment and stock relating to patient care.
Training, professional and personal development:
Thepost holder will participate in any training programme implemented by thepractice as part of this employment, such training to include:
Training requirements will be monitored by yearly appraisal and will bein accordance with practice requirements.Personal development will be encouraged and supported by thepractice. It is the individualsresponsibility to remain up to date with recent developments.
Taking responsibility for own development, learning and performance anddemonstrating skills and activities to others who are undertaking similar work.
Participate in the education andtraining of students of all disciplines and the induction of all members of thepractice staff where appropriate.
Develop and maintain an in housetraining programme reporting to the practice manager.
Maintain continued education byattendance at courses and study days as deemed useful or necessary forprofessional development, ensuring revalidation requirements are met.
If it is necessary to expand the roleto include additional responsibilities, full training will be given.
Develop and maintain a PersonalDevelopment Plan.
As well as the clinical team, there is a need to work closely withreception to ensure the smooth running of the practice, reporting any problemsencountered to the relevant person and ensuring everyone is aware of thedifferent roles within the clinical team.
There is also the need to establish andmaintain good liaison with other surgeries and agencies, including secondarycare
Meetings:
It will be necessary to attend and contribute to various practicemeetings as requested. Attendance atsuch meetings will be mandatory.
