Overview
The Oxford Haemophilia and Thrombosis Centre (OHTC) is a designated comprehensive care centre primarily serving Oxfordshire, with patients referred from surrounding counties for specialist input. The DVT and Anticoagulation (AC) services are nurse-led with consultant support. The thrombosis team includes haematology doctors and a team of nurses. The research team, venous thromboembolism (VTE) prevention team and haemophilia service also operate within the same centre.
Responsibilities
- Use expert knowledge and clinical judgement to decide whether to refer patients for investigations and discuss potential diagnoses with the wider multidisciplinary team (MDT).
- Decide on and carry out treatment, including advice on prescribing medicines or referring patients to an appropriate specialist.
- Assess individuals, families and populations holistically using a range of methods, including physical assessment, history taking and specialist health needs assessments.
- Ensure continuity of care and assess and evaluate, with patients and/or carers, the effectiveness of treatment and care provided, making changes as needed.
- Work autonomously within a wider health care team and as a lone practitioner; work across professional, organisational and system boundaries to develop and sustain partnerships and improve health outcomes and healthcare delivery.
- Work at an advanced level of practice within the specialist area, autonomously as well as within a multidisciplinary team; demonstrate autonomous decision making in comprehensive history taking and physical assessment of patients.
- Plan, prioritise and manage complete episodes of care, delegating and referring as appropriate to optimise health outcomes and resource use; provide direct support to patients and act as a key worker where this model exists.
Qualifications
- Registered nurse (part 1, 8, 12 or 15 of the register depending on area of practice).
- Postgraduate level study (HEI level 7).
- Teaching/Assessment/Mentoring qualification; demonstrable experience caring for patients with chronic care needs; knowledge and experience in specialty/bleeding disorders; evidence of continuing professional development.
- Previous experience of formal/informal teaching of patients and staff; appropriate UK professional registration.
