A leading mental health trust in Hertfordshire is seeking a Team Leader to manage a multi-disciplinary Integrated Care Team. The role involves providing day-to-day management and leadership to nurses, social workers, and occupational therapists while ensuring the highest standards of practice. Candidates should have relevant nursing qualifications and experience in mental health support. This position aims to deliver integrated care, assisting service users with both physical and mental health needs in the community.
The Integrated Care Team (ICT) aims to make healthcare more preventative - this service supports service users with multiple long term or complex conditions to remain at home rather than going into hospital or residential care. Referrals are accepted from primary care, ambulance service, A&E and secondary care.
This role is within the Integrated Care Team in HPFT which is a nurse-led service offering specialist mental health support to wider primary care services within Hertfordshire Community Trust and local Primary Care Networks. ICT works alongside GP’s, matrons, district nurses, OT’s and physiotherapists to name a few but also supporting hospices, care homes, other primary level specialist services (such as heart failure nurses) and adult care services. We support by assessing, providing interventions, monitoring any medication changes, signposting or escalating where necessary for both physical and mental health needs.
Our referral criteria is not age restricted but based on frailty and home based care, supporting people with both physical and mental health needs at a primary care level and short term (up to 12 weeks). The team covers East and North Hertfordshire and based in 6 localities. Alongside mental health nurses the team has psychology and OT input.
To work in collaboration and to ensure the highest standards of practice and service delivery. To work as part of the Mental Health Directorate to deliver the services required under the National Service Framework for Mental Health.
To provide day -to- day management, leadership and supervision to a multi- disciplinary Integrated Care Team.
The Team Leader will manage nurses, social workers, occupational therapists and other staff, ensuring that the needs of service users are met through the effective deployment of these staff. They will also maintain a clinical role and/ or a caseload dependent on sector need.
The Team Leader will contribute at a local level to the development and implementation of policies and procedures.
