Job summary

A domiciliary care agency in Greater London is looking for a Team Leader to oversee service user assessments, ensure care plans are updated, and manage a team of care workers. The role involves communication with care coordinators, supervising worker activities, and ensuring client coverage at all times. Responsibilities

  • Complete all service user assessment, including risk assessments.
  • Routine monitoring and updating of care plans.
  • Participate in an on-call system for emergencies.
  • Communicate with care coordinators and hire/distribute care workers.
  • Ensure supervision and mentorship of field-based care workers.
  • Assign replacement care workers if needed.
  • Maintain accuracy of written reports and data.
  • Ensure all client visits are covered.
  • Conduct orientation visits for new care workers.

Job Description Of Team Leader
  • Complete all service user assessment, including risk assessments as part of the assessment process, for new service users.
  • As required by the company's policies, routinely monitor, review, and update the care plan for the client.
  • To participate in a rotating system of being on call in case of emergency.
  • Should communicate often with care coordinators regarding hiring, distributing care workers, and developing care & support.
  • To keep a close line of communication open with the Care Coordinator and Registered Manager.
  • Support all field-based carer workers and ensure care workers in the field are observed, supervised, and mentored.
  • When a current care worker reports that they won't be able to make to attend to a call, assign a suitable replacement care worker.
  • To make sure that all written reports, administration, and data kept in a service user’ homes are accurate and current.
  • Make sure all client visits are always covered.
  • Conduct new care worker orientation visits with clients.
  • Immediately notify the care coordinators of any alterations to a care worker or service user's schedule.
