About the role

We’re recruiting a compassionate and enthusiastic Team Lead to join our caring and progressive community‑based nursing team at KCHFT. This role focuses on holistic rehabilitation and therapy, enabling nurses to spend more time with patients and promote self‑management.

Responsibilities
  • Lead the community nursing team, making independent clinical decisions while supporting colleagues.
  • Provide and coordinate holistic community rehabilitation and end‑of‑life care.
  • Drive to patient homes and sites, ensuring safe and effective care delivery.
  • Support development of staff, including those without specialist District Nurse qualification, through trainee posts and SPDN qualification pathways.
  • Implement and promote self‑management strategies to improve patient outcomes.
  • Adhere to NHS car lease provision and claim mileage costs after probationary period.
Qualifications
  • Registered nurse (UK) with experience in community nursing or related clinical setting.
  • Full UK driving licence (or full licence issued by an EU country) – essential for the role.
  • Experience in independent clinical decision‑making and leadership.
  • Desire to develop advanced clinical skills or SPDN qualification – traineeships welcome.
  • NHS Graduate Guarantee support is available for newly qualified nurses.
Eligibility

Open to nurses currently employed within Kent and Medway NHS Trusts and the Kent & Medway Integrated Care Board, plus student and newly qualified nurses.
