Job responsibilities
  • Drive to patients in community settings; full UK driving licence required.
  • Make independent clinical decisions and manage complex caseloads.
  • Promote self‑management and improve patient lives using the named nurse model.
  • Support trainees and candidates progressing towards advanced qualifications such as District Nurse or SPDN.
  • Provide excellent patient experience, signposting to alternative services where appropriate.
  • Collaborate with interdisciplinary teams and use IT systems for documentation.
  • Maintain up‑to‑date clinical practice and adhere to evidence‑based protocols.
  • Support the NHS Graduate Guarantee and training pathways.
Qualifications, training and experience
  • Level 6 Degree or equivalent qualification, or equivalent knowledge plus a diploma in nursing.
  • Registered with the Nursing and Midwifery Council.
  • Experience managing complex caseloads and mentoring others.
  • Customer‑focussed approach with excellent communication skills.
  • Knowledge of NHS developments, National Service Frameworks and NICE guidance.
  • IT system and database competency.
  • Attention to detail and ability to spot potential problems.
  • Evidence‑based practice and understanding of health care worker remit.
Working environment
  • Full, valid UK driving licence and independence to travel.
  • Support for workplace adjustments under the Equality Act 2010.
