About The Role Support Worker -Assisted Discharge

Location : Midland Metropolitan University Hospital

Salary : £, per annum

Hours : per week

Contract : Fixed term contract for months

As a Support Worker, you’ll be a lifeline for people who may be feeling vulnerable after a hospital stay. Whether it’s helping with shopping, checking in with a phone call, or simply being a friendly face, your support will help people live more independently. It’s more than just a job - it’s a chance to give back, grow your skills, and be part of a team that truly cares.

What will a day in the life of a Support Worker involve?
  • Providing practical and emotional support, including welfare calls, shopping, and home visits.
  • Supporting adults after a hospital stay to help them recover safely at home.
  • Responding to referrals from clinicians and community health professionals.
  • Assessing needs and following a personalised support plan for each service user.
To be a successful Support Worker, what will you need?
  • A kind, calm and confident approach with great people skills.
  • Right to work in the UK. Unfortunately we are unable to offer sponsorship for this role.
  • The ability to work independently and as part of a team.
  • A full UK manual driving licence and willingness to drive service vehicles.
  • Basic IT skills and a commitment to following procedures.
