Job description
Overview

The post holder will provide Support, give Time to an allocated group of service users and promote their recovery. To assist the Care Coordinator and the Multi-disciplinary team to assess, plan, implement and evaluate individual care plans. The purpose of this post is to work as part of a team focusing on the direct needs of service users working across boundaries of care and organisations and use the agreed set of values and skills to underpin their day-to-day work. To provide a flexible working pattern that meets the needs of service users.

Responsibilities
  • Be responsible for contributing to the Step-Down Process of service users in the community. This includes attending and actively participating in MDTs, training sessions, care plan review meetings and care coordination as required.
  • Under supervision from the MDT, carry out prescribed care as per an agreed care plan to meet the individual needs of service users.
  • Assist in the development and preparation of activities that conform to the goals of an agreed care plan.
  • Positively promote independent living within the community and make activities responsive to the needs of the service user.
  • Provide support with daily living activities which promote social inclusion and recovery.
