A leading mental health service provider in Manchester is seeking a Support Worker to provide direct support to service users. In this role, you will work collaboratively with the multidisciplinary team to assist in care plans, promote recovery, and help individuals live independently. The post holder will provide Support, give Time to an allocated group of service users and promote their recovery. To assist the Care Coordinator and the Multi-disciplinary team to assess, plan, implement and evaluate individual care plans. The purpose of this post is to work as part of a team focusing on the direct needs of service users working across boundaries of care and organisations and use the agreed set of values and skills to underpin their day-to-day work. To provide a flexible working pattern that meets the needs of service users.
