Overview

We have an exciting opportunity for a Stroke Community Rehabilitation Support Worker to join our established Integrated Community Stroke Service to deliver rehabilitation into various community settings. The role is part of a multidisciplinary team that includes occupational therapists, physiotherapists, speech and language therapists, clinical psychologists and assistant practitioners. Rehabilitation is currently delivered 8am‑5pm, 5 days a week, including bank holidays, with future weekend working possibilities.

Responsibilities
  • Provide day‑to‑day therapeutic care to stroke patients within the community, supporting patients to live their optimum life and achieve independence.
  • Deliver therapy treatment plans, working collaboratively within a multidisciplinary framework and providing compassionate, realistic and timely support to a diverse patient group and their families.
  • Assess patients presenting with significant physical health care needs and support stroke recovery through personalised rehabilitation and exercise programmes.
  • Communicate with confidence and empathy, forming strong relationships with patients, families and colleagues.
  • Work flexibly across South Nottinghamshire, supporting evolving service needs and maintaining high quality integrated care.
Qualifications & Experience
  • Full time Stroke Community Rehabilitation Support Worker role, motivated, passionate and committed to patient‑centred care.
  • Experience or training in stroke rehabilitation or allied health disciplines that enable delivery of evidence‑based care.
  • Excellent communication and interpersonal skills, reliable, resourceful and approachable.
  • Full UK driving licence and vehicle for business use is required; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
