Job description

An exciting opportunity to join our Cardiac Rehabilitation Team has arisen for a band 5 staff nurse, to cover maternity leave, for a 1 year fixed term post.

Under the direct supervision of the Cardiac Rehabilitation Clinical Nurse Specialist and Senior Nurses within the team, you will delivery a high quality service to people with coronary heart disease.

You should have a good understanding of coronary heart disease, including myocardial infarction, angina, coronary angioplasty, coronary bypass surgery and valve surgery.

You will work closely with cardiac rehabilitation, heart failure cardiac rehabilitation, cardiology clinical nurse specialists and cardiology consultants.

You will provide inpatient and outpatient assessment to people with coronary heart disease, identifying risk factors for health behaviour change, whilst working to national, local and trust guidelines.

Main duties of the job
  • Working within cardiac rehabilitation, delivery evidence based practice to people with coronary heart disease.
  • To assess people diagnosed as inpatients with coronary heart disease under cardiac rehabilitation remit, providing education and risk factor modification advice
  • To provide advice and education for safe self-management of medicines
  • To provide education and advice on exercise and activity post coronary heart disease events
  • Under supervision from senior members of the team, participate in the exercise & education sessions within cardiac rehabilitation
  • To work independently, within their own limitations of experience and competence
