A leading healthcare organization in England is seeking a dedicated professional to provide services to individuals experiencing homelessness and substance use issues. In this role, you will undertake assessments, manage a caseload, and deliver interventions focused on harm reduction. You will work collaboratively within the housing options team to provide comprehensive support and care, ensuring the best outcomes for clients. High-quality care and a commitment to staff wellbeing are at the core of our mission.

To provide a dedicated, comprehensive service to people experiencing homelessness, being at risk of homelessness and having substance related problems, or being at risk of relapse back into substance use and have a history of, or are at risk of disengaging from services. To also work closely with and provide advice and guidance to colleagues within the housing options team to integrate substance use intervention into care plans, increasing engagement and ongoing support, where appropriate.

We support individuals, families and communities who are affected by addiction, crime, mental health or need treatment for their sexual health.
