Overview

Coordination of care to maximize independence through rapid multidisciplinary assessment and support. You will provide essential moving and handling equipment, liaise with adult social care teams about additional support, and deliver short‑term rehabilitation. This role drives forward our Community Therapy services in Surrey Downs, requiring strong organisational and time‑management skills and a passion for holistic, patient‑centred care. You will develop new ways of working to improve quality and foster a “one team” culture.

Key Responsibilities
  • Co‑lead Occupational Therapy with existing Band 7 OT and wider team.
  • Act as a key member of the multidisciplinary team in the assessment and management of a caseload of clients.
  • Plan, deliver and evaluate the treatment needs of clients with a variety of complex social and health care needs.
  • Participate as an active member of the MDT in developing and delivering high‑quality, innovative services to the local population.
  • Promote effective teamwork within Surrey Downs Health and Care, delegating activities appropriately for patient and service benefit.
  • Collaborate closely with other sectors in the organisation, with potential rotation opportunities.
  • Achieve job expectations through hands‑on clinical practice, education, training, and research.
Qualifications and Experience

You must have broad clinical experience, including frailty and falls, and enjoy the challenges of providing rehabilitation to people in their own homes.

You will be supported by the Clinical Lead and experienced Band 7 physiotherapists, with professional OT support from senior OTs in the organisation.

As part of your role you will be responsible for training and supervising Band 6 Occupational Therapists, Rehabilitation Assistants, and Physiotherapists, and you will develop the OT service within the team.

You will also commit to the ‘Fair Share’ model of student placement development.
