What you will do:
  • Manage complex adult mental health cases and coordinate seamless hospital discharge processes.
  • Conduct statutory Care Act tasks, including Care Act assessments, reviews, and care planning.
  • Work effectively alongside multi-disciplinary professionals, attending ward rounds and collaborating with Housing and brokerage services.
  • Handle Court of Protection applications and take part in Mental Health Tribunals.
  • Carry out Safeguarding Enquiries and perform duty work efficiently.
  • Travel around and beyond the borough as needed, requiring you to have a valid driver’s license and your own transport.
  • Thrive in a fast-paced environment by proactively managing and prioritising workloads, ensuring urgent cases receive the attention they require.
