Job description

Integrated Learning Disabilities Service. We are sourcing candidates from various channels for this role. We are looking for experienced qualified social workers to join the Integrated Learning Disabilities Assessment & Care Management Team. The team is part of an integrated health and social care service providing a wide range of support to people with learning disabilities and their carers. Our integrated team consists of community nurses, psychologists, occupational therapists, physiotherapists, speech and language therapists, psychiatrists and an employment service. As a result, we can offer our service users a holistic approach to support.
