What you will do:

• Assist young people aged 14 and over in developing skills and independence necessary for their future adult lives.
• Conduct assessments of young people and adults under The Care Act to tailor bespoke support plans.
• Provide comprehensive assessment, support planning, monitoring, and reviews both face-to-face and over the phone, ensuring that vulnerable people are safeguarded and risks are managed.
• Collaborate with individuals and carers to identify personal outcomes and work in tandem with them to achieve these goals using a strengths-based approach.
• Conduct annual reviews to continually meet the developmental needs of clients.
• Participate in a duty rota to guarantee consistent service delivery.
• Collaborate effectively as part of a dynamic, multi-disciplinary team dedicated to improving lives.

Requirements of the Social Worker:

• Hold a degree or equivalent qualification in Social Work.
• Must have current registration with Social Work England.
• Possess significant frontline experience within UK Social Services working as a qualified Social Worker.
